Is Vasectomy Reversal Always Successful?

For some couples, vasectomy reversal is a necessary first step toward building a family together. How It Works

When a vasectomy is performed, the vas deferens (the tube that carries sperm away from the testes) is divided. A vasovasostomy, or vasectomy reversal, is the surgical reconnection of the vas deferens.

During the procedure (which is performed under general anesthesia), a small incision is made in the scrotum. The vas deferens is isolated and dissected microscopically. The vas deferens is reconnected using multiple layers of micro suture, and is then placed back into its normal position, with a small drain inserted on each side. The incision is closed with absorbable sutures that will dissolve on their own. The drains are removed the day after surgery. The procedure itself is painless, and the discomfort afterward is about the same as the discomfort after a vasectomy.

Factors Affecting Success

If you’re considering a vasectomy reversal, it’s important to understand the various factors that can affect the success of your procedure. From the surgeon’s point of view, a vasovasostomy is considered a success if the vas deferens is reconnected. But the successful reconnection of the vas deferens doesn’t necessarily mean that sperm will be present in the ejaculate. In general, the longer it’s been since your vasectomy was performed, the lower your chances of a positive semen analysis.

That said, success rates for vasectomy reversal are high. If your vasectomy was performed fewer than five years ago, there is a greater than 95 percent chance of sperm in your ejaculate after a vasovasostomy. If your vasectomy was between five and ten years ago, there is a 90 percent chance of a positive semen analysis. If your vasectomy was over 10 years ago, that chance is between 80 and 90 percent.

Keep in mind also that a successful vasectomy reversal doesn’t necessarily guarantee a pregnancy. Both partners must be fertile in order for pregnancy to occur; and sometimes undiagnosed fertility problems in the female partner can prevent conception, even when sperm is present in the male’s ejaculate. If pregnancy does not occur after a vasectomy reversal and a positive semen analysis, it may be necessary to seek treatment from a fertility specialist.
